Add Questions to Templates
Build your template by adding questions with different response types, organizing them in sections, and marking required fields.
Before You Start
- You need Manager or Supervisor permissions
- You must be creating or editing a template
Steps
Step 1: Open the template editor
Either:
- Create a new template via New Template
- Or click on an existing template and select Edit
Step 2: Add a question
- In the right-side menu, click to add a question
- The question appears in the template structure
- Enter your question text

Step 3: Select the response type
Click on the question to configure it:
For Inspection Templates:

| Numeric field | Numbers only | Measurements, counts, temperatures |
| Single selection | Choose one option | Categories, states (Good/Bad) |
| Multiple selection | Choose multiple options | Multi-condition checklists |
| Yes / No / N/A | Three-option choice | Quick verifications |
| Date and time | Date/time picker | Timestamps, schedules |
| Multimedia | Photo/video upload | Evidence, documentation |
| Location | Select from locations | Site-specific checks |

| Instruction | Display text only | Guidance, no response needed |
For Procedure Templates:
- Only Yes / No / N/A is available

Step 4: Configure options (for selection types)
For Single Selection or Multiple Selection:
- Click on the question
- Add the options operators can choose from
- Reorder options if needed
Example: "Equipment condition" with options: Excellent, Good, Fair, Poor

Step 5: Mark as required (optional)
- Select the question
- Enable the Required toggle
- Operators must answer this question to complete the inspection

Step 6: Organize with sections
Group related questions:
- Add a Section from the right menu
- Name the section
- Drag existing questions into the section, or add new questions within it
Sections help operators understand the logical grouping of questions.
Step 7: Reorder questions
- Drag and drop questions to reorder them
- Move questions between sections as needed
- The order in the template is the order operators will see
Step 8: Delete questions
- Select the question
- Click the delete option
- Confirm deletion
Note: Deleted questions cannot be recovered after saving.
Tips
- Use clear, specific question text — avoid ambiguity
- Match response type to the expected answer (don't use text for yes/no questions)
- Mark critical safety checks as required
- Use instructions to provide context before complex sections
- Keep questions concise — detailed guidance can go in the description
- Test the flow by reviewing the template as an operator would see it
